Abstract

Background

It is well documented that Egypt has the highest prevalence of hepatitis C virus (HCV) infection in the world. The recent development of highly effective direct acting antiviral drugs (DAAs), has opened the possibility of treating and curing HCV infection in the Egyptian population on a large scale.

Methods

A screening demonstration project was implemented in southern Egypt in and around the city of Luxor. Free screening and if indicated, treatment, was offered to those 16 years or older for anti-HCV antibodies (anti-HCV) and hepatitis B surface antigen (HBsAg) using third generation enzyme immunoassays (Enzygnost® Anti-HCV and HbsAg). Statistical methods included estimation of odds ratios (OR) and 95% confidence intervals (95% CI).

Results

There was a large turnout of 67,042 persons who were screened in a 12-month period starting in June 2016. Thirty-one thousand nine hundred sixty-four males (47.7%) and 35,074 females (52.3%) were screened with a mean age of 43.6 ± 14.3 years. Nine thousand seven hundred one patients (14.5%) were positive for anti-HCV and 2950 (4.4%) for HBsAg. Prevalence of anti-HCV was significantly higher in males than females (19.67% vs.9.73% OR = 2.27; CI 2.2 to 2.4; p < 0.001) and the same for HBsAg (6.2% vs. 2.8% OR = 2.3; CI 2.2 to 2.5; p < 0.001). The prevalence of anti-HCV was significantly associated with age (p < 0.001), ranging from between 1 and 4% in individuals below the age of 40 years, then increased steadily to 42% at age 60 followed by a precipitous decline in age specific prevalence.

Conclusions

The results showed unanticipated patterns in the Luxor area of anti-HCV and HBsAg by age and gender in contrast to previous reports on this unique HCV epidemic in Egypt. Moreover, the level and rate of turnout, cost, and other logistical issues, provided essential information for effective planning, design, and evaluation methods for larger national mass screening and treatment programs.

BACKGROUND AND OVERVIEW

Dr. Miller has an international track record of epidemiologic research, teaching and service.

OVERVIEW

Research overview: Dr. Miller is recognized internationally for his contributions to the epidemiology and control of schistosomiasis, especially in Egypt.  His work is cited in WHO technical documents on schistosomiasis control.  At the request and support of WHO, Dr. Miller, representing UH, prepared a report which was submitted to the Director General of the United Nations who presented the report to the United Nations General Assembly on the evaluation of schistosomiasis control measures.

Dr. Miller was the team leader and principal architect for the design of the $45 million Schistosomiasis Research Project, and principal architect of its epidemiologic field studies, the largest ever completed on schistosomiasis in Egypt or elsewhere in the world, past or present.

The prevention of Hepatitis C virus transmission in Egypt and has been Dr. Miller’s prime focus and endeavor since 2007.

Dr. Miller is co‑discover of the largest epidemic of hepatitis C virus in the world today which is also located in Egypt (Kamel, M.A. and Miller, F. D. 1992. High HCV Prevalence in Egyptian Blood Donors. Lancet 340:427). This discovery has led to the prevention of HCV transmission through blood transfusion in Egypt and other ongoing control programs. We were the first also to publish on the epidemiology of hepatitis C virus in an Egyptian community and found that 18% of that population had been infected (Kamel MA, Miller FD et al. 1994. The epidemiology of S. mansoni, hepatitis B, and hepatitis C infection in Egypt. Annals of Tropical Medicine and Parasitology 88 (5) 501-509).

An article appeared in the Daily Star Egypt regarding a key note address given at the American University Cairo by Dr. Miller on HCV.  The link is given below.

The publication in the Proceedings of the National Academy of Science is a landmark epidemiological study on the incidence of hepatitis C virus in Egypt that was published last year in 2010. This publication was in turn carried by a number of news agencies around the world. The following are links to various news reports of our publication.

 The Egyptian Mummy Project

Dr. Miller obtained a $1 million grant from the National Geographic Society for the Egyptian Mummy Project and obtained a state of the art portable CT scanning machine for the Egyptian Supreme Council of Antiquities, Dr. Zahi Hawass.  This machine is currently located directly behind the Cairo Museum and was used for the scientific study of the Egyptian Mummy Tutankhamen in which he appeared in an internationally televised documentary special.

Department Chair: In 2000, Dr. Miller designed and implemented a new department, the Department of Public Health Sciences and Epidemiology at the John A. Burns School of Medicine, University of Hawaii which obtained national accreditation in May 2002 one year ahead of the usual 3 year period needed to start and accredit a new MPH program. During this period, he also recruited 7 new faculty members, hired one additional administrative staff,  renovated 3 laboratories from complete disuse which are now used for epidemiologic infectious disease research (HIV, West Nile virus, and Dengue Fever), built a computing center (also used by JABSOM), recruited and graduated students, won extramural funding (to date: $1.47 million + $250,000 pending contract completion), enlarged the PhD graduate faculty to 21 from 9, and reopened the PhD graduate program for admission (it had been closed for more than 4 years), and started a new MPH graduate program in social and behavioral health sciences. This was accomplished without funding or resources from the School of Medicine.

Dr. Miller’s other epidemiological research include the first ever epidemiologic investigations of Egypt’s Zabaleen communities (an ethnic class that collects solid waste). The resulting publication resulted in the World Bank funding for improving these communities.  A similar study was done in Thailand.

He has also conducted epidemiologic studies on chronic cardiovascular diseases at the NiHanSan Honolulu Heart Program in Hawaii.

Vietnam

He was CoPI with Dr. R. Yanagihara on a dengue fever epidemiological proposal to PDVI for $2 million. Although the proposal was awarded, delays in processing the award by the University resulted in losing this investigation. A great deal of effort had been made to win this award including two site visits to Vietnam.

Colleagues in Hawaii, the first population based antibiotic resistanceMRSA – patterns was published in the USA.  This study included all antibiotic sensitivity testing in the entire state of Hawaii over a 5 year period.
