Sabbatical/Professional Improvement Leave

    • Submit memo to request
      • Make sure you indicate how it will benefit the Department
      • Also indicate how duties will be covered

Workers' Compensation (APM A9.720)

  • Employee requirements
  • For WC Coordinator (Nate)
    • Fax the following completed forms to First Insurance Company (545-3120) and Suzette Leong (956-2304)
      • UH Form 79 - Report of Work-Related Injury/Illness (pages 8-9)
      • UH Form 42 - Computation of Average Weekly Wages for Temporary Disability Payments (page 15) - to be completed by
    • UH Form 41 is to be kept with the file.
  • If time-off is required for treatment, UH Form 83 needs to be completed
    • Supervisor fills in date and time left and signs
    • Medical provider completes Part III
    • Supervisor fills in date and time returned and signs
    • Give form to WC Coordinator (Nate)

 

Terminations/Resignations

  • Employee(s) need to submit Letter of Resignation to their supervisor
    • Original Letter is given to Personnel Officer
  • Send Original PNF to UH Payroll
  • Fax PNF to ERS (586-1731)
  • For Civil Service Employees
    • Send to OHR-Civil Service Section
      • DPS Form 7
      • 4 prior years of vacation & sick leave balances
  • Prepare and fax EC-1 to EUTF (586-2161)
    • If Employee is unavailable for signature, then type in "Prepared by Department"
  • Form G-2
    • APT
      • Send original G-2 to UH Payroll with a copy of last Leave Report
      • Send a copy of G-2 to ERS via State Messenger Service
    • Civil Service Employees
      • Send Original G-2 to UH Payroll
      • Send copy of G-2 with DPS Form 7 & leave records for past 4 years to OHR - Civil Service Section
      • Send a copy of G-2 to ERS via State Messenger Service
  • BOR Online Reporting
  • Hybrid Reimbursements
    • Employee should call ERS (586-1735) with their questions and to get instructions on how to get their reimbursements
