Although this RIO has members who are University of Hawai'i at Manoa students, faculty, or employees, the RIO is independent of the University which is not responsible for the RIO's contracts, acts or omissions.

**Important**

(1) Judo Club/Team at the University of Hawaii Membership Application Form

(2) Judo Club/Team at the University of Hawaii Release and Waiver of Liability and Indemnity Agreement Form

(3) Judo Club/Team at the University of Hawaii Photo/Video Release Form

(4) Judo Club/Team at the University of Hawaii Rule of Engagement Form

(5) Submit response to following questions (Question Form)

  • Please complete and sign the membership application form, the release and waiver of liability and indemnity agreement form, the photo/video release form, rule of engagement form, and quesiton form.
  • Club members will be required to submit a signed membership application form, a Release and Waiver of Liability and Indemnity Agreement form, a photo/video release form, rule of engagement form, and quesiton form before working out with the club.
  • Due to liability concerns, club members who do not wish to sign the membership application form, the release and waiver of liability and indemnity agreement form, the photo/video release form, rule of engagement form, and question form may not participate in the club.

(6) Submit Proof of Valid Judo Membership (See Example)

  • Judo membership is required for participation in practice sessions. All members and visitor must have USA Judo, USJF, and/or USJA membership. Please provide a printed copy of your membership and bring your Judo membership card at every practice.
  • To Join USA Judo (Primary Membership) (see below for step-by-step instruction on How to Join USA Judo)
  • To Join USJF (Secondary Membership) (see below for step-by-step instruction on How to Join USJF)
