(5) Submit response to following questions (Question Form)
- Please complete and sign the membership application form, the release and waiver of liability and indemnity agreement form, the photo/video release form, rule of engagement form, and quesiton form.
- Club members will be required to submit a signed membership application form, a Release and Waiver of Liability and Indemnity Agreement form, a photo/video release form, rule of engagement form, and quesiton form before working out with the club.
- Due to liability concerns, club members who do not wish to sign the membership application form, the release and waiver of liability and indemnity agreement form, the photo/video release form, rule of engagement form, and question form may not participate in the club.
(6) Submit Proof of Valid Judo Membership (See Example)
- Judo membership is required for participation in practice sessions. All members and visitor must have USA Judo, USJF, and/or USJA membership. Please provide a printed copy of your membership and bring your Judo membership card at every practice.
