Conceptual Design

I understand that I am voluntarily participating in a Wise Web Wizards (WWW) usability study. This study will evaluate the Resume Index service found on the Information at Computer Science (ICS) Department's home page (http://www.ics.hawaii.edu) at the University of Hawaii (UH).  My participation includes completing background and post-usability questionnaires about my current technical background and my opinions with regards to the usability of the Resume Index service.  I am one of approximately 3 participants involved in this usability study.

I understand that I can withdraw from this study at any time and have the information provided in my questionnaires removed in entirety from this study.  This decision will not affect me in any manner.

I understand that all gathered data will be kept confidential to the extent provided by law, and that any and all references to information about my data or me will be kept anonymous to the extent provided by law.
