If OCLC has a partial record for a photocopy of a UHM thesis and there is no record in Voyager:
 
a)         Create the UHM Prelim record with MFHDs and ITEMs as usual
b)         Count paper stats as a prelim
c)         Send paper copy⁄copies to Preservation for binding
