1) Going outside after you get the yellow key is not the last step. The
red key is in one corner of the yard. Ignore the double doors, and return
inside.
2) Go to one of the red "restricted area" doors and open it. Clean up
inside, push the button on the wall and wait until the room stops
turning. Get inside both security stations and deactivate the force
fields.
3) Kill everything in both cell wings and go to the right wing. Use the
four-way switch to open the last door on the left and go inside. There
will be a girlie poster on the wall; go through and when you reach the end
bomb the wall twice. The tunnel afterwards leads to where you want to go.
Also, when you go underwater where the sub is, you'll see a large red door on the wall. It's a fake. In fact if you no-clip beyond it (with Atomic), and go the right direction you'll actually wind up inside the sub :). Otherwise, there is only void space beyond it. Just to let you know since this question has been asked frequently.
*If you still don't get it :(, there is a tunnel behind the yellow canisters in the left wing that leads to the Death Row cell. There's a message in the corner only readable by night goggles (hidden in a nook inside the Death Row gear room).*
In the movie "Shawshank Redemption," Andy Dufresne (Tim Robbins) is a prisoner. He's the model inmate so the guards pretty much let him do what he wants. In his cell he has these large posters of women stars (Marilyn Monroe, etc.). While carving his name on the wall one day he noticed the wall crumbled. He decided to dig his way out of prison using the poster to hide his work. Over time (less than 20 years) he dug a narrow hole to a crawlspace behind the wall. One night he snuck out, reached the crawlspace, knocked a hole in the sewage pipe, crawled through it for 500 yards (eg. 1500 feet), and into the drainage ditch outside the prison (as told by his buddy Red, Morgan Freeman).
Everyone was surprised when the hole was discovered behind the poster, Red and the warden included. Andy got off scot free, the paper had an expose' on the prison's corruption, a guard was arrested, and the mean warden blew his brains out with a gun. Go see the movie-- Shawshank prison does rehabilitate!
Many gamers on alt.games.duke3d have asked how to get into the Duke Burger kitchen because it's blocked by a forcefield. I've found a new secret way to get inside (and you don't even need the blue key!). Go across the street from the drive in window and face away from the alley and the wall graffiti. Between that red and yellow line on the wall is a secret opening with goodies. You'll need the jetpack to get there, because the hole is more than your jumping height.
After you collect everything you will notice an opening to the kitchen. Without alerting the aliens (I use F7 to see) throw a pipebomb in and blow it. Then get inside and finish them off. If you want to clean up the rest of the place (pun intended) hit the switch by the door to deactivate the forcefield.
You can also get to the secret cranny from _inside_ the kitchen. How? Just align yourself correctly with the arrow keys on your keyboard and jump from the "Wash Your Hands" counter till' you get it right. Good luck!

So you come to the launch control room, and find you are stuck! Luckily you have hints. There are three places where you can find the launch codes.
A small note: the screen behind the 4 way switch in the launch control room shows Duke Burger from episode 3. It is being attacked by the same aliens from "Independence Day," since Duke was released after the movie was in 1996.
