 Background and Overview 

Research overview: Dr. Miller is recognized internationally for his contributions to the epidemiology and control of schistosomiasis, especially in Egypt.  His work is cited in WHO technical documents on schistosomiasis control.  At the request and support of WHO, Dr. Miller, representing UH, prepared a report which was submitted to the Director General of the United Nations who presented the report to the United Nations General Assembly on the evaluation of schistosomiasis control measures. 

He was the team leader and principal architect for the design of the $45 million Schistosomiasis Research Project, and principal architect of its epidemiologic field studies, the largest ever completed on schistosomiasis in Egypt or elsewhere in the world, past or present.

He is co‑discover (Kamal et al. 1992) of the largest epidemic of hepatitis C virus in the world today which is also located in Egypt.  This discovery has lead to the prevention of HCV transmission through blood transfusion in Egypt and other ongoing control programs. 

Dr. Miller obtained a $1 million grant from the National Geographic Society for the Egyptian Mummy Project and obtained a state of the art portable CT scanning machine for the Egyptian Supreme Council of Antiquities, Dr. Zahi Hawass.  This machine is currently located directly behind the Cairo Museum and was used for the scientific study of the Egyptian Mummy Tutankhamun in which he appeared in an internationally televised documentary special.

In 2000, Dr. Miller designed and implemented a new department, the Department of Public Health Sciences and Epidemiology at the John A. Burns School of Medicine, University of Hawaii which obtained national accreditation in May 2002 one year ahead of the usual 3 year period needed to start and accredit a new MPH program. During this period, he also recruited 7 new faculty members, hired one additional administrative staff,  renovated 3 laboratories from complete disuse which are now used for epidemiologic infectious disease research (HIV, West Nile virus, and Dengue Fever), built a computing center (also used by JABSOM), recruited and graduated students, won extramural funding (to date: $1.47 million + $250,000 pending contract completion), enlarged the PhD graduate faculty to 21 from 9, and reopened the PhD graduate program for admission (it had been closed for more than 4 years), and started a new MPH graduate program in social and behavioral health sciences.

OTHER
Dr. Miller’s epidemiological research also includes the first ever epidemiologic investigations of Egypt’s Zabaleen communities. The resulting publication resulted in the World Bank funding for improving these communities.  A similar study was done in Thailand.

He has also conducted epidemiologic studies on chronic cardiovascular diseases at the NiHanSan Honolulu Heart Program in Hawaii.

In collaboration  with colleagues in Hawaii, the first population based antibiotic resistance – MRSA – patterns were published.  This study included all antibiotic sensitivity testing in the entire state of Hawaii over a 5 year period.

I have lived and or worked in seven countries, including Egypt, Iran,  Micronesia, other Pacific nations, and Southeast Asia.

Egypt:
1975 – 1979. Designed and conducted a country wide study of schistosomiasis (bilharzia) which included 20,000 Egyptians living in villages in the Nile Delta, Middle and Upper Egypt and in New Nubia.  This study took 4 years and received international attention because it proved that the Aswan High dam did not cause an increase in schistosomiasis in Egypt as had been widely predicated in western countries.

1981 – 1982. Designed and conducted schistosomiasis studies in Qena on 5,000 rural villagers.  This study showed that schistosomiasis in Upper Egypt was declining.

1983.  UNDP funded project to conduct an epidemiological investigation on the impact of waste collection by Egyptian zaballeen.  This was the first epidemiologic report ever published in the medical science literature on this community and was the justification for large investments by the World Bank to improve the conditions of the zaballeen communities.

1989 to 1998:  The Schisotosomiasis (Bilharzia) Research Project in Egypt. This $48 million dollar 10 year project was the largest single research and control project for the research and control of bilhariza in Egypt or elsewhere, now or historically, ever completed.

I was the principal architect of this project both in concept and design.  It took 7 years to obtain the funding and support for this project. The project result was a major contribution to the current control of bilhariza in Egypt today, a major contribution to the science and research of bilhariza, and a major contribution to the development of young Egyptian scientists.  It was a significant contribution to Egypt.

1992 – 1994:  With my Egyptian colleagues, we were the first to discover and publish the now well known epidemic of hepatitis C virus in Egypt. We also published the first community population based studies confirming the widespread nature of the epidemic and that the infection increased by age. With my colleagues in epidemiologic modeling of infectious diseases, we have submitted a publication on estimating the incidence of HCV in Egypt. An abstract of this work was accepted by the 2010 International Congress on Infectious Diseases.

From June 2013 to July 2014 I was in Cairo Egypt where I have a non-compensated appointment at the American University of Cairo and and similar appointment at the Cornell Medical Collage in Doha, Qatar.
